PLEASE READ FIRST: In order to be eligible for dispatch by RL Trucking, LLC or
its affiliates, all of the following documents must be submitted as a complete packet. You may chose to submit the packet in person or by mail to our mailing address. Do not submit packets by facsimile.
Only complete packets will be accepted. The State of Alaska and Federal Department of
Transportation require, by law, that we maintain complete, accurate and legible files for each owner operator. Please remember that it is your responsibility to ensure this information is up-to-date and accurate. Should any of the information in your packet change after it has been submitted, please notify our office and supply the new documentation immediately.
Remember, incomplete packets will be discarded and you will not be eligible for dispatch. No exceptions.
One copy of each of the following documents is required:

Alaska Commercial Driver's License (for each driver)
+Current Medical Card (for each driver)
+Social Security Card (for each driver)
+DMV-furnished
driving record (not more than one year old)
+IRS
Form W-9: Request for Taxpayer Identifcation Number
+Current Alaska Business License
+Current
Workers Compensation Insurance Policy certificate
+Dated Drug/Alcohol Consortium Membership letter
+Current
Tax Clearance provided by
the State of Alaska Department of Labor
+State of Alaska DMV registration for all equipment to be used on a job
+Certificate of Insurance
showing minimum $1,000,000 general libaility coverage
